Applying
Prior to applying for any business certification programs, DSBO highly recommends that you first attend our weekly Certification Training.

To apply for either or both business certification programs, download and complete the application and forms listed below. 

DBE, SBE and M/WBE:

• Uniform Certification Application (24 pgs), (complete all sections including the shaded areas, and return with supporting documents on the “Checklist”)

DBE only:

• If you want to apply for DBE only, please go to the Colorado Department of Transportation Web site to submit an application directly through the state.
  

Once the application and supporting documents are fully completed, your application packet can be mailed to the address below:

Renewing and Work Code Changes
Businesses looking to renew their SBE, M/WBE and/or DBE certification need to fill out the forms listed below and return them to:
